Buy a regular qwerty keyboard with removable keys (most cheap ones are) and re-arrange the keys...

But of course after physically rearranging the keys you would need to use a key remapping program like

Remap or Disable Keyboard Keys with Key Mapper

so that you could reconfigure the layout on the software side as well.

You can use Window's built in language settings through control panel and add US-Dvorak and then just select it in the taskbar (in 7 at least forget in earlier ones)

yeah the keys on most cheap keyboards are just clipped on, you can simply gently remove the keys with a screwdriver or some other tool and rearrange them.

I made the switch to Dvorak a couple weeks ago and I just use the keyboard without moving the keys cause it discourages looking at the keys which makes you faster
